This page informs you of our policies regarding the collection, use and disclosure of Personal Information (defined below) we receive from users (or, as the case may be, legally authorized agents’ minor wards) of lastdrawer.com.

We use your or your wards’ Personal Information only for the purposes stated in the Terms and Conditions accessible at lastdrawer.com. Specifically, we have developed an on-line mechanism that allows you to gather, edit, add to, store, and share your or your child’s/ward’s health information online. With lastdrawer.com, you can control your own and your child’s/ward’s health records, and you can also share such health information with family, friends, healthcare professionals, mobile phone applications, health-related devices, and online tools. By using lastdrawer.com, you agree to the collection and use of information in accordance with this Privacy Policy. Unless otherwise defined in this Privacy Policy, terms used in this Privacy Policy have the same meanings as in our Terms and Conditions.

Information Collection And Use

While using lastdrawer.com, we may ask you to provide us with certain personally identifiable information that can be used to contact or identify you. Personally identifiable information may include, but is not limited to, your name, email address, postal address and phone number (“Personal Information”). You will also be required to execute the Authorization in order to utilize our services.

lastdrawer.com asks you to enter an identifier and password to sign in. We recommend having more than one sign-in credential for your account, to help ensure you can access your data if you lose one of them. The first time you sign in to lastdrawer.com, you will be asked to create a lastdrawer.com account (the “Account”). To create the Account, you must provide personal information such as name, date of birth, e-mail address, postal code and country/region. Depending on which features you use, you may be asked for additional information for that feature (such as creating an emergency profile or fitness goal).

We will use the e-mail address you provide when you create your Account to send you an e-mail requesting that you validate your email address, to include in sharing invitations you send through lastdrawer.com, and to send you notifications, such as e-mail notification that information is available to add to your or your child’s/ward’s records.

Your Account allows you to manage one or more health records, such as the ones you create for yourself and your family members. You choose what information to put in your records. Examples of the types of information you can store in a record include:

  • fitness-related activities such as aerobic sessions
  • measurements such as blood glucose and blood pressure
  • discharge summaries from hospitalizations
  • lab results
  • medications
  • health history

lastdrawer.com allows an account to contain multiple health records. This feature enables, for example, family health managers to create and manage records for family members.

When you create a record, you become a custodian of that record. As a custodian, you decide what level of access to grant other users of lastdrawer.com. lastdrawer.com creates a fixed list of each access or change by it and its users, which lastdrawer.com keeps as a full history of the record. You can view and update records you are custodian of and can examine the history of access and changes to those records.

Sharing records with other lastdrawer.com users

By default, you are the custodian of any records you create. You may invite additional people to be custodians. Each custodian can add and remove other custodians and users who can view and modify the record. Some of the information stored in the records you manage may be highly sensitive, so you need to consider carefully with whom you choose to share the information. A record may have multiple custodians.

A key value of lastdrawer.com is the ability to share your health information with people and services who can help you meet your or your family’s or ward’s health-related goals. For example, you can share health information from records you control:

  • to co-manage the health of a family member
  • to use it with other health-related products and services
  • to consult with your healthcare provider
  • to provide fitness information to coaches and trainers

You can share information in a health record you are custodian of with another person by sending a sharing invitation e-mail through lastdrawer.com. If the person accepts your sharing invitation and has or creates an Account, you have given him or her access to that information. You can specify how long they have access (custodian access does not expire but, like all sharing access, it can be revoked at any time) and whether they can modify the information in the record. Each person who accepts a sharing invitation can grant the same level of access that the person has.

You can also choose to grant custodian access to other persons, such as your spouse, for any record of which you are a custodian. Custodian access is the broadest level of access, so you should think carefully before you grant custodian access to a record. Every custodian of a record has the same access to the record, including accessing, modifying, deleting, and sharing all the information in the record. A custodian can also revoke access to a record from any other custodian of the record, including you.

The United States enables participating providers to obtain reports about whether the information they send to a record is used. This feature supports the United States’s “meaningful use” objective of the HITECH Act, which provides incentives for healthcare providers to send their patients copies of their medical information electronically. Providers that participate can get reports that include:

  • A number that the provider uses to identify the patient within the provider’s system
  • An indication if the user took one of the “qualifying actions” in lastdrawer.com during the time period of the report (but no information about what action)

“Qualifying action” currently includes activities such as viewing, downloading, or transmitting health information via email in lastdrawer.com. You can turn off reporting for your records.

lastdrawer.com may use aggregated information from lastdrawer.com to improve the quality of lastdrawer.com and for marketing of lastdrawer.com (for example, to tell potential advertisers how many lastdrawer.com users live in the United States). This aggregated information is not associated with any individual account.


You can delete any health record that you are a custodian of by signing in to your Account and editing a record’s profile. If other users had any level of access to that record, the record no longer appears in their accounts. lastdrawer.com deletes the record from all users. We wait 90 days before permanently deleting the record information in order to help avoid accidental or malicious removal of your health information.

When a person moves a piece of health information to the trash, custodians may still view it there, and can restore it or delete it permanently from there at any time.

Permanently deleting health information removes it from the trash. Once someone permanently deletes an item, we cannot restore it. lastdrawer.com adds an entry in your record history noting the name of the person who permanently deleted information and the date the deletion occurred.

Log Data

Like many site operators, we collect information that your browser sends whenever you visit lastdrawer.com (“Log Data”). This Log Data may include information such as your computer’s Internet Protocol (“IP”) address, browser type, browser version, the pages of our Site that you visit, the time and date of your visit, any changes you may make, times and dates of log-ins, the time spent on those pages and other statistics.

Other Information

To help operate and improve the service, we also collect information about how you interact with our services, including the browser you are using, your IP address, location, cookies or other unique IDs, the pages you visit and features you use. We combine this with other users’ information to get an overall view of how the service is used.


Cookies are files with small amounts of data, which may include an anonymous unique identifier. A cookie is a small text file that a web page server places on your hard disk. It is not possible to use cookies to run programs or deliver viruses to your computer. You agree that You are solely liable for all content You upload and distribute. In addition, You agree that You will not upload, post, transmit, transfer, disseminate, distribute, or facilitate distribution of any content, including text, images, video, sound, data, information, or software, that:

Uses this Site for any purpose in violation of local, state, national, or international laws;

Uses this Site as a means to distribute advertising or other unsolicited material to any third party;

Personal Use, Rights and Responsibilities

You agree not to share information that others could use to harm You. Parents and Guardians agree to be aware of and help exercise control over content posted by and activities of their children.

Client and Agent Obligations

Client and/or Agent agrees to provide any information necessary to enable us to create an account for You or, as the case may be, to access healthcare information about You, the Client, or Your children or wards. If You are a Guardian of a child, then We will require proof of legal guardianship to request the child’s documents. You agree to provide all information necessary to complete such requests and to hold lastdrawer.com harmless if the provider does not respond to requests for any reason.

Termination and Cancellation

lastdrawer.com reserves the right, at its sole discretion, and without any obligation to do so, to review, remove and delete prohibited content at will and without notice.

lastdrawer.com reserves the right, at its sole discretion, to ban or terminate access to services by Clients and/or Agents who violate the account Terms and Conditions of Use. You are personally liable for any charges that You incur prior to termination.

lastdrawer.com reserves the right to change, suspend or discontinue all or any aspects of the Site at any time without prior notice.

If You revoke your consent to this Agreement or Your contract for services provided by lastdrawer.com, advanced written notice of thirty days shall be required. Upon written request from You, We will remove all of Your or Your child’s information from the Site; provided, however: (i) to the extent such information has been used or transmitted to third parties, such information shall not be required to be returned to the Agent; and (ii) We may choose to archive such information for Your future access should You choose to utilize Our services at a later date.

Fees and Payment

A valid credit card number, expiration date, and card security code is required for all purchases. If lastdrawer.com is unable to charge the full cost of Your purchase to the credit card You provide, We may cancel Your order or request an alternate form of payment. You understand that the prices for services may be changed from time to time, but that the Site will reflect current prices. You agree that all purchases are non-refundable except as required by law. You are solely responsible for providing complete and accurate billing and contact information to lastdrawer.com. We may suspend or terminate services if any fees are past due.

If You choose the auto renewal feature on Your account, lastdrawer.com will automatically charge the account You provide at the time of renewal without further notice to You. You must cancel the auto renewal feature before the charge date, or the fee charged will be non-refundable.

Client/Agent is responsible for all taxes. Last Drawer will charge tax when required to do so. If Client/Agent is required by law to withhold any taxes, Client/Agent must provide lastdrawer.com with an official tax receipt or other appropriate documentation.


You agree that you will indemnify lastdrawer.com against any damages, losses, liabilities, judgments, costs or expenses (including reasonable attorneys’ fees and costs) arising out of a claim by a third party relating to materials You have posted or other actions taken by You on the Site.

Intellectual Property

The marks “lastdrawer.com,” “lastdrawer.com,” and the lastdrawer.com five-part emblem are trademarks of Unified Initiatives, L.L.C. The design of the lastdrawer.com Site is protected by U.S. and international copyright laws. Unlike a doctor or health plan, lastdrawer.com is not regulated by the Health Insurance Portability and Accountability Act (HIPAA), a federal law that establishes data confidentiality standards for patient health information. This is because Last Drawer does not store data on behalf of healthcare providers; instead, our primary relationship is with the user.

Although Last Drawer is not covered by HIPAA, we are committed to user privacy and have strict data security policies and measures ensuring ensure that You control access to Your information. You choose who views or adds information to Your profile, and You can revoke access at any time.

Information Sharing

If You share Your information with others, You can view a list of who has access to Your information and You can revoke sharing privileges at any time. When You revoke someone’s ability to read Your health information, that party will no longer be able to read Your information on the Site, but may have already seen or retained a copy of the information.

You can approve access for some service providers to view and copy Your health information. If a service provider accesses Your health information and stores a copy of Your information, that copy will be governed by that service provider’s privacy policy. Others at that facility – like an on-call doctor – may be able to view Your information. lastdrawer.com is not responsible for the content, performance, or privacy policies of third-party service providers.

Some of third-party service providers will be covered by federal and state health privacy laws (such as the Health Insurance Portability and Accountability Act, or “HIPAA”), and those laws will govern how they may use and share Your information. HIPAA requires (as does lastdrawer.com) that You must authorize these providers to send information to Your lastdrawer.com account. With that authorization, You also give them permission to send certain especially sensitive types of health information (such as mental health or substance abuse records) that are protected by federal and state laws and require special authorization. All entities or business associates covered by HIPAA are contractually required to comply with HIPAA’s rules related to collection, use, and sharing of Your information. All other third-party service providers are contractually required to abide by the lastdrawer.com Developer Policies, which require that they comply with strict privacy standards for how they collect, use, or share Your information.


You agree that any dispute or claim (including personal injury claims) related to your use of the lastdrawer.com Site is subject to the exclusive jurisdiction of the appropriate court in Arizona and will be adjudicated pursuant to the laws of the state. This agreement also applies to anyone claiming on your behalf.

The terms of this Agreement are in addition to the TERMS AND CONDITIONS OF USE POLICY you have with us. This Disclosure and Agreement (“Agreement”) is the contract which covers your and our rights and responsibilities concerning the service offered to you by lastdrawer.com, which permits you to electronically request medical documents online.

In this Agreement, the words “you,” “your,” and “yours” refer to those persons who request records through and use the services of lastdrawer.com, including any joint account holders or any person authorized by an account holder to use lastdrawer.com’s services. The words “we,” “us,” and “our” Unified Initiatives, L.L.C. doing business as lastdrawer.com. The words “document” or “documents” refer to the following individually and/or collectively: Authorization to Release Medical Records, Consent Form, and Immunization Exemption or any other document lastdrawer.com sends via paper or other means now or in the future. By using lastdrawer.com’s services, each of you, jointly and severally, agree to the terms and conditions of this Agreement, and any amendments. You agree to review the agreement periodically to be aware of all such modifications, and your accessing or using the Site constitutes your acceptance of the agreement as it appears at the time of your access or use.

Electronic Signature (e-Signature): You consent and agree that your use of a key pad, mouse or other device to select an item, button, icon or similar act/action while using any electronic service we offer; or in accessing or making any transactions regarding any document, agreement, acknowledgement, consent, term, disclosure, or condition constitutes your signature, acceptance and agreement as if actually signed by you in writing (“e-Signature”). Further, you agree that no certification authority or other third party verification is necessary to validate your e-Signature; and the lack of such certification or third party verification will not in any way affect the enforceability of your s-Signature or any resulting contracts between you and lastdrawer.com. You understand and agree that your e-Signature will be legally binding and each such transaction will be considered to have been authorized by you.
